Nevada is considering extending the power to initiate mental health holds to paramedics. This is an appropriate and needed change but there are very important issues involved. Everyone in the community needs to understand the effects on EMS, patients, liability and basic human rights and freedoms that this change entails.

Unstable tachycardias represent a special set of problems for EMS. Pitfalls include differentiating primary cardiac issues from mimics such as sepsis and use of antidysrhythmics such as amiodarone. We also talk about calling a STEMI in the field, bigeminy and there's a bonus rant about EKG telemetry that degenerates into talking about the 70s TV show EMERGENCY! which, to be honest, isn't uncommon around here. So enjoy!
